WHY WE WORK

Getting to thriving
We believe in a future where every person has all they need in order to live their own definition of thriving. We’re working to build a future in which all the systems that contribute to an individual or family’s health and wellness are integrated and working together.
Centering community
We believe in centering individuals and communities who are furthest from power and privilege. Lived experience is integrated into the decision making structure of local community Commons, and the larger Co-Thrive strategy.
Dismantling biased systems
We believe in actively disrupting systems of oppression to make thriving accessible for all. Where we live and how much money we make should not determine our health outcomes.
Building trust in relationships
We believe in the power of strong relationships to enhance health and well-being. Our coaching model fosters personal connections and community support, ensuring that everyone has a trusted advocate by their side.
Continuing to learn and evolve
We believe in continuous learning and integration. We know driving innovation requires failing forward, constantly learning, establishing feedback loops to refine our processes and then enhancing our strategies.
Paying for collaboration
We believe in the value of investing in cross-sector collaboration. By providing financial support for collaborative efforts, we encourage partnerships and teamwork that drive innovation and achieve shared goals.

We believe in upstream investment. By proactively allocating resources toward preventive measures and foundational improvements, we address root causes and create sustainable, long-term benefits.
Our Approach
A Whole Health Approach to Care
We’re building a holistic approach to health inclusive of physical, mental, spiritual, and socioeconomic well-being as defined by individuals, families, and communities.
Coaches
Our Commons Coaches stand on the shoulders of the Promotores de Salud model, developed in the 1960s. They develop trust-based longitudinal support relationships with the person in the center.
Community "Commons"
The “Commons” is a formal community-based collaborative between community leadership, physical healthcare, mental healthcare, public health, social services, and community based organizations. It has a formalized operations and governance model.
Health-Related Social Needs (HRSNs)
We’re working to address non medical social and economic needs that affect a person’s ability to thrive, in addition to their clinical needs.
Upstream Investments
We’re focused on allocating resources and energy to address the root causes of health and life issues, rather than treating symptoms.
